Consent for Grooming and Wellness Services

I, the undersigned owner (or authorized agent), hereby authorize Bagshot Row Pet Wellness Center and its staff to perform grooming, bathing, trimming, nail clipping, ear cleaning, teeth brushing, and any other requested wellness services on my pet.

Matted Coat Acknowledgment and Consent

I understand that severely matted coats can present health risks to my pet. I consent to the removal of mats using the most appropriate method as determined by the groomer. I release Bagshot Row Pet Wellness Center from liability for skin irritation, minor cuts, or coat changes resulting from necessary dematting.

Emergency Contact & Medical Authorization

In the event of an injury or emergency, I authorize Bagshot Row Pet Wellness Center staff to immediately transport my pet to a veterinary clinic and call me (or my emergency contact) on the way.

Injury Procedure Acknowledgment

I understand and agree that if my pet is injured, staff will immediately stabilize and transport my pet to a veterinarian while contacting me. Bagshot Row Pepper Farm will pay for the initial exam only. I am responsible for all additional costs.

Liability Insurance

I acknowledge that Bagshot Row Pet Wellness Center maintains general liability insurance. Any claim is limited to the insurance coverage available. I will not seek recovery from personal assets beyond the insurance policy.

General Release of Liability

I hereby release, indemnify, and hold harmless Bagshot Row Pet Wellness Center, its owners, employees, and agents from any and all claims, damages, or liabilities arising from grooming services, except those caused by gross negligence or willful misconduct.

I confirm my pet is current on vaccinations (especially rabies) and is free from contagious conditions to the best of my knowledge.
